Preventing Muscle Loss During GLP-1 Treatment
Protect strength and lean mass during GLP-1–assisted weight loss with individualized protein, resistance exercise and body-composition monitoring.

Weight lost is not exclusively fat. As with other effective weight-loss approaches, some lean mass can be lost. The goal is not to prevent every change in lean mass, but to preserve muscle, strength, function and adequate nutrition—especially in older adults or people losing weight rapidly.
Four pillars of muscle preservation
- Adequate protein: needs vary with body size, kidney health, age and diet. W8MD individualizes the target rather than using one number for everyone.
- Progressive resistance exercise: major muscle groups two or more days per week may be appropriate, beginning at a safe level.
- Enough total nutrition: excessive restriction can worsen fatigue and nutrient shortfalls.

Who needs extra attention?
Older adults, people with sarcopenia risk, repeated falls, frailty, kidney disease, restrictive eating, very rapid loss or low baseline protein intake need individualized planning. Sudden weakness deserves evaluation rather than automatically being blamed on treatment.
Evidence: the STEP 1 body-composition analysis found reductions in fat and lean mass while body composition improved. A 2025 multidisciplinary advisory emphasizes adequate nourishment, resistance training and lean-mass preservation. See sarcopenia.
