What patients should know

Prior authorization commonly requires the correct diagnosis, current measurements, comorbidities, previous treatment, contraindications, requested product and plan-specific criteria.

Medical visit

W8MD determines whether treatment is clinically appropriate. Insurance participation for the visit must be confirmed for the exact plan.

Medication benefit

The pharmacy benefit separately controls formulary status, prior authorization, quantity limits, preferred products and patient cost.

Self-pay alternative

Patients without usable benefits can compare transparent self-pay programs, traditional prescription options and GLP-1 pathways.

A practical verification process

  1. Use the exact insurance card and current benefit year.
  2. Confirm whether W8MD is in network or whether the plan includes out-of-network benefits.
  3. Ask separately whether anti-obesity medication is covered by the pharmacy benefit.
  4. Identify authorization criteria, deductible, coinsurance and copay.
  5. Compare the expected insurance responsibility with the complete self-pay cost.
  6. Keep denial notices and appeal deadlines.

W8MD insurance and self-pay options

W8MD is not currently in network with Oxford, Healthfirst, MetroPlusHealth or UnitedHealthcare. If a patient has usable out-of-network benefits, W8MD may accept them after verification. Otherwise, the patient can use a self-pay option.

  • $150 per month: includes the medical weight-loss visit and, when medically appropriate and available for in-office dispensing, traditional prescription diet pills such as phentermine and/or topiramate.
  • $120 per month: includes the medical weight-loss visit and prescription management; the prescription is sent to the patient’s pharmacy of choice, and pharmacy charges are separate.
  • Self-pay semaglutide: from $59.99 per week and up.
  • Self-pay tirzepatide: from $69.99 per week and up.

For some patients, the difference between an insurance-based visit and self-pay may be approximately $40–$50, and a plan copayment may approach that difference. This is only a general comparison; deductibles, coinsurance, out-of-network reimbursement and medication benefits vary.
