1. Is the clinical visit covered?

Verify network status, specialist rules, copay, deductible and referral requirements.

2. Are anti-obesity medicines included?

Some employer and individual plans exclude the category even when other prescriptions are covered.

3. Which product is preferred?

A plan may require a preferred medicine, step therapy or documentation of prior treatment.

4. What is the patient’s cost?

Approval does not eliminate copays, coinsurance, deductibles, quantity limits or pharmacy restrictions.

Before the appointment

  1. Call the pharmacy-benefit number on the insurance card.
  2. Ask whether medications for obesity or chronic weight management are included.
  3. Request the current formulary and prior-authorization criteria.
  4. Ask about Wegovy, Zepbound, Saxenda, Qsymia, Contrave and orlistat individually.
  5. Ask whether step therapy, a preferred pharmacy or mail order is required.

What prior authorization may require
