At a glance: Orlistat is a nonstimulant gastrointestinal lipase inhibitor. Prescription Xenical is labeled for obesity management including weight loss, maintenance and reducing regain; lower-dose Alli is sold over the counter for eligible adults.

Choosing a medication is not a popularity contest. W8MD reviews weight history, blood pressure and pulse, current medicines, pregnancy potential, sleep, eating pattern, prior response, adverse effects, insurance rules, budget and long-term goals before discussing orlistat.

Medical screening

Medication reconciliation, contraindication review and assessment of weight-related conditions.

Whole-plan care

Protein-forward nutrition, activity, sleep and preservation of lean mass—not a prescription alone.

Follow-up

Monitor response, tolerability, vital signs when relevant, plateaus and maintenance planning.

How it differs from appetite medicines

Orlistat reduces absorption of some dietary fat rather than acting as a stimulant or GLP-1 medicine. Gastrointestinal effects—including oily spotting, urgency and loose stools—are more likely when meals contain more fat.

Vitamins, medicines and health history

A multivitamin containing fat-soluble vitamins is generally taken at a separate time according to product directions. Discuss warfarin, cyclosporine, levothyroxine, seizure medicines, diabetes medicines and kidney history. Chronic malabsorption syndrome and cholestasis are contraindications.

Is it a reasonable fit?

It may interest patients who prefer a nonstimulant, noninjection option, but dietary pattern, gastrointestinal tolerance and interactions matter. Compare all choices in our NYC medication chart.
