What is Obesity or Overweight?

For adults, a “body mass index” (BMI) of 30 and over in general is considered to be obese although BMI may not be an ideal tool to determine the risk. It is far more important to understand the distribution of the weight, body fat composition etc. to understand the risks associated with Obesity.

Understanding the risks

You could be in nomal BMI range and but can still have metabolic syndrome, pre-diabetes or even type 2 diabetes , and other diseases associated with weight such as cholesterol abnormalities. This is especially true if you have weight in the wrong areas such as the belly and upper part of the body.

BMI is used because of the simplicity and for most people, it tends to correlate with their amount of body fat.

•An adult who has a BMI between 25 and 29.9 is considered overweight.
•An adult who has a BMI of 30 or higher is considered obese.

It is important to remember that although BMI correlates with the amount of body fat, BMI does not directly measure body fat. As a result, some people, such as athletes, may have a BMI that identifies them as overweight even though they do not have excess body fat.

Waist to hip ratio

Other methods of estimating body fat and body fat distribution include measurements of body composition analysis, skinfold thickness and waist circumference, calculation of waist-to-hip circumference ratios, and techniques such as ultrasound, computed tomography, and magnetic resonance imaging (MRI).

The ideal waist to hip ratio for women should be 0.8 or lower and for men should be 1 or lower. That means, you should not have a big tummy or upper body weight for reducing the metabolic risks associated with weight.

Income and Weight Gain

Overweight and obesity affect people in all income ranges. But people who live in low-income areas may face even greater barriers to eating healthy foods and being active than other people. High-calorie processed foods often cost less than healthier options, such as fruits and vegetables. There also may be few safe, free, or low-cost places nearby to be active on a regular basis. These factors may contribute to weight gain.

What is polycystic ovaries and how do you know if you might have it?

For many overweight or obese woman, a condition called polycystic ovarian syndrome (PCOS), also called Stein Levinthal Syndrome, is an underlying issue that not only makes it harder to lose weight but also can affect many other things such as irregular or reduced menstrual cycles, excess hair growth, and fertility related to issues.

Did you know that the underlying issue that causes PCOS is actually insulin resistance which might be also causing you to gain weight

How common is Polycystic Ovarian Syndrome in NYC?

It is estimated that polycystic ovarian syndrome affects about 8-10% of women of reproductive age. It is very common. However, the underlying issue called insulin resistance that is one of the major causes of PCOS affects up to 71% of the population which means many woman may have subtle signs of insulin resistance leading to subclinical polycystic ovaries that are not yet diagnosed.

The infertility rate with polycystic ovaries is very high. these women usually will have difficulty getting pregnant – and usually require treatment to improve chances for pregnancy.

What are the signs and symptoms of polycystic ovaries in New York City?

Polycystic ovaries can be clinical leading some symptoms and signs or subclinical meaning there are no overt symptoms or signs. When it is clinical, you may experience one or more of the following symptoms.

Menstrual: abnormal menstruation, absence of menstruation, heavy menstruation, irregular menstruation, short and light menstruation, or spotting
Weight: obesity, overweight, or weight gain
Skin: acne or oily skin
Excess Hair and other signs of hyperandrogenism: PCOS causes high levels of masculinizing hormones(hyperandrogenism). The most common signs of hyperandrogenism are acne and hirsutism (male pattern of hair growth, such as on the chin or chest), but it may produce hypermenorrhea (heavy and prolonged menstrual periods), androgenic alopecia (increased hair thinning or diffuse hair loss), or other symptoms. Approximately three-quarters of women with PCOS (by the diagnostic criteria of NIH/NICHD 1990) have evidence of hyperandrogenemia.
Metabolic syndrome: This appears as a tendency towards central obesity and other symptoms associated with insulin resistance. Serum insulin, insulin resistance, and homocysteine levels are higher in women with PCOS.

What are the risks associated with PCOS in NYC?

A diagnosis of PCOS suggests an increased risk of the following:
    • Endometrial hyperplasia and endometrial cancer (cancer of the uterine lining) are possible, due to overaccumulation of uterine lining, and also lack of progesterone resulting in prolonged stimulation of uterine cells by estrogen.
    • Insulin resistance/Type II diabetes. A review published in 2010 concluded that women with PCOS have an elevated prevalence of insulin resistance and type II diabetes, even when controlling for body mass index (BMI).
    • Gestational diabetes. PCOS also makes a woman, particularly if obese, prone to gestational diabetes.
    • High blood pressure, in particular if obese or during pregnancy.
    • Depression and anxiety. The risk of depression and anxiety also increased with PCOS.
    • Dyslipidemia – disorders of lipid metabolism — cholesterol and triglycerides. Women with PCOS show a decreased removal of atherosclerosis-inducing remnants, seemingly independent of insulin resistance/Type II diabetes.
    • Cardiovascular disease, with a meta-analysis estimating a 2-fold risk of arterial disease for women with PCOS relative to women without PCOS, independent of BMI.
    • Strokes, Weight gain, Miscarriage, Sleep apnea, particularly if obesity is present.
    • Non-alcoholic fatty liver disease, again particularly if obesity is present.
    • Acanthosis nigricans (patches of darkened skin under the arms, in the groin area, on the back of the neck).
    • Autoimmune thyroiditis.
  • Infertility and difficulty losing weight are also commonly seen with PCOS
Early diagnosis and treatment may reduce the risk of some of these, such as type 2 diabetes and heart disease.
